MAIN DUTIES AND RESPONSIBILITIES:
· Performs direct patient care:
o Answer patient call signals, signal lights, bells, or intercom systems to determine patients' needs.
o Renders bed bath, afternoon care, oral care and other comfort measures for category 1 & 2 patients.
o Bowel preparation / Enemas (soap suds, saline and 3 way cleansing enema only).
o Proper positioning of category 1 & 2 patients.
o Daily changing of linen.
o Diaper change for incontinent or bedbound patients.
o Supply, collect, empty bedpans. Empty urine bag.
o Preparing patients for sleep by rendering afternoon care and oral hygiene.
o Offers empty clean bedpan, urinals and commode.
o Post mortem care and transports cadaver to the morgue, if needed.
o Prepares /assists patient for his/her meals, serves and removes food trays, orally feeds a patient with no companion.
o Follow proper procedures for lifting and moving patients.
o Transfers patients in and out of bed, to and from a wheelchair / stretcher inside the room.
o Walks patient if ambulatory or thru the use wheelchair to go around the hospital corridor, doctor's clinic, to the chapel and to other department for diagnostic purposes, except for patients with any oxygen device and intravenous fluid.
o Assembles supplies and prepare equipment for the patient as instructed by the nurse.
For NA II trained:
In additional to the NAP functions listed:
Record height or weight of patients.
Performs vital signs taking (including Pain Scale) for all category 1&2 patients.
Collects data for patient intake and output before the end of the shift.
Collect specimens, such as urine, feces, or sputum. Follows hospital policy on correct labeling.
Performs other bedside procedure as directed by the Staff Nurse.
Reports any abnormal findings to the Staff Nurse.
Assist the nurse in performing comfort measures such as bed bad, proper positioning/ repositioning to category 3 & 4 patients.
· Relays patient's request, observation findings and complaint to staff nurse or nurse manager.
· Picks up medicine from Pharmacy except for narcotics and Intravenous chemotherapy medications
· Picks up and return borrowed/ordered supplies and equipment from Central Supply or Sterilization Unit, as directed by the nurse.
· Brings specimen such as urine, stool, sputum, pleural fluids, peritoneal fluids, paracentesis fluid, spinal fluids, and biopsy specimens to the Laboratory Department or other diagnostic centers. Ensure that all specimens are properly received by authorized diagnostic personnel.
· Ensures availability of portable oxygen tanks and portable compress air tanks at all times. Brings empty tanks to Central Supply for refilling when necessary.
· Performs housekeeping activities:
o Care of Utility
Performs surface cleans equipment in the unit such as cardiac monitor, infusion pumps, push cart, commodes, flushing trays at the beginning and end of any shift.
Defrosts and/or cleans the unit refrigerator for medications and feedings.
Records refrigerator temperature every shift and reports variance in reading to the nurse manager.
o Disposes content of used patient equipment/ supply very shift or as needed and upon patients discharge.
o In the Operating/ Delivery Complex
Performs terminal cleaning of all operating suites and delivery room after each case.
Performs surface cleaning of all equipment and fixtures.
Position or hold patients in position for surgical preparation.
Transports used/ dirty instrument to the Central Sterilization Unit using the designated cart for soiled instrument.
Thoroughly cleans, dries and encloses instrument/ items in designated pouches following standards, infection control protocols and manufacturer's recommendation.
Performs sterilization of instrument or item following standards and manufacturer's recommendation.
Checks functionality of sterilization machine before use. Reports any concerns or issues to the Head Nurse/Charge Nurse
Receives and transports sterilized instrument to the Operating/ Delivery Complex.
Prepares cleaning agent following manufacturer's recommended procedure.
· Returns used patient care devices to its designated storage area upon patient's discharge.
· In the Medical-Surgical Unit:
Performs Daily Inventory of the following: E-Cart, Medicine and Supplies every shift.
Performs Daily Defibrillator and Laryngoscope Checking.
Performs item replenishment thru Post Charges and STR of ICU Medicines and Supplies.
Performs Posting of Patient's Diet thru Bizbox.
Helps NOD in answering telephone calls and receiving messages except for physician's order.
Helps in facilitating Discharge Process such as:
Return unused supplies and medicines to CSSR and Pharmacy Department respectively.
Performs suctioning (Oral, ET, Tracheostomy) upon proper training and supervision of a staff nurse.
Assist the nursing attendant in removing soiled linen and preparing new set of beddings for new admission.
· Maintains cleanliness of the nursing stations, medication room and utility room.
· Demonstrates effective infection control practices conscientiously (ex. waste disposal, correct hand washing, personal protective equipment).
· Attends staff meeting and participates in quality improvement initiatives of the unit.
· Performs other duties that may be assigned.
WORKING CONDITIONS
· Rotating shifts as well as holiday, work during inclement weather and on call will be required.
· This job role requires regular walking to various locations around the hospital.
· Regularly exposed to the risks of blood borne and airborne diseases.
· Wears personal protective equipment as required.
· Job requires lifting and carrying of light to medium objects.
QUALIFICATIONS:
a. Basic Life Support Training, within 6 months upon hiring.
b. College graduate with 1 year of any health related / technical course.
c. Completed training course for NAP II personnel.
d. With the ability to effectively communicate to patients and customers both in oral and written medium.
e. Has good moral character, pleasing personality and exhibits professionalism at all times.
f. Can work harmoniously with the healthcare team members.

1. Assists in the preparation of materials ie fistula kits, bloodlines, subkits, subclavian kits, etc. Ensure that all items required for dialysis treatment are in good condition prior to actual treatment,
2. Assists Dialysis Nurse in the preparation of the dialysis machine prior to treatment such as priming of bloodlines and dialyzer, and during pre- and post-dialysis weight
3. Assists in patient admission procedures such as taking of vital signs, preparation of documents
4. Assist in recording data in the NephroWeb to ensure that all pertinent clinical and patient information are kept updated
5. Ensures all specimens are appropriately packaged, labeled, sealed according to policies and procedures relating to shipment of blood or body fluid specimens and potentially hazardous material, prepares specimens for shipment and monitors delivery and submission of specimens and receipt of results
6. Reports malfunctioning dialysis equipment or apparatus such as Bp app, glucometer, thermometer, pulse ox, Glucometer to Charge Nurse, Team Leader or Unit Head
7. Ensures all patient stations, including machines and dialysis chairs, are clean and free of blood spills and other foreign matter.
8. Monitors vital signs as per doctor's order; assess patient condition during treatment
9. Assist with the coordination of patient transportation if necessary and perform other patient care related errands such as getting medication from the refrigerator, getting blood for transfusion, getting dialyzers from the Dialysis Technicians, etc.
10. Assists nurses in receiving patients into the treatment area, transporting them into the dialysis station
11. Monitor patient admission for renal update, encode and submit necessary patient information to PRDR
12. Develops and maintains positive relationships through effective and timely communication with all customers
