What You'll Do – Your Role
- Conduct in-person, preventive cardiology and chronic disease-focused visits in Assisted Living and Skilled Nursing settings
- Monitor patients' chronic cardiac conditions daily, using remote technology
- Collaborate with primary care providers, other cardiologists, and our internal team.
- Document clinical notes and treatment plans with attention to detail
- Educate patients, families and staff on heart health, prevention, and wellness

What You Bring
– The Ideal Candidate, Active Nurse Practitioner License (Florida). Strong background or interest in Cardiology or Geriatrics Experience, working in long-term care settings preferred. Proactive care.
