1.0 Duties and Responsibilities:

A. Patient Care

1. Initiates hemodialysis of patients

· Cannulates the AV Fistula

· Opens and connects AV shunt to bloodlines

1. Carries out doctor's prescriptions/therapeutic managements

2. Computes for and monitors ultra-filtration rate

3. Observes for signs and symptoms of complications and administers appropriate management and care

4. Refers to Supervising/Attending Nephrologist for problems

5. Terminates the dialysis procedure

6. Explains the procedures and treatments given to patients

7. Ensures the safety of patients during the procedure.

B. Unit Management

1. Assists the Dialysis Nurse in maintaining adequate supplies, instruments, equipment and drugs of the unit

2. Assists the Dialysis Nurse in maintaining a clean, comfortable, safe working area.

C. Staff Development

1. Attends seminars, in-service training, lectures, initiated by the health care facility

2. Attends unit meetings.

D. Role in Patient and Family Rights Responsibility

1. Actively support Patient's Rights and Responsibilities policy and procedure of the organization.

2. Ensure that all patients are aware and educated about their rights and responsibilities in accordance to the hospital policy.

3. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. Functions as liaison between administration, patients, physicians and other healthcare providers.

4. Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.

Educational Qualification and Licensures:

1.1 Diploma or Bachelor Degree in Nursing in an accredited college or university.

1.2 Valid PRC ID

1.3 Current Basic Life Support (BLS) Certificate, with Advance Cardiac Life Support (ACLS) and IVT preferred.

The Staff Nurse will coordinate care with consultants, residents, physicians-on-duty, and other health professionals to provide quality nursing service to patients with renal conditions.

• Ensure proper endorsement and monitoring of patients before, during, and after hemodialysis treatment.

I. Duties and Responsibilities:

1. Oversees the preparation of the delivery system, dialysate bath, and dialyzer and confirms that all mandatory alarm tests on the dialyzer and equipment are performed.

2. Reviews physician orders for dialysis patients, collects pre-treatment dialysis data, and reviews patient records prior to dialysis.

3. Conducts pre-dialysis patient assessment including obtaining patient vital signs and assessing the patient' s vascular access, laboratory findings, and the patient' s general health.

4. Verifies that patients are taking all prescribed medications and performs medication reconciliation.

5. Documents findings of patient assessments and interventions and advises providers of any significant change in the patient's condition and other pertinent information.

6. Discusses patient concerns and answers questions relevant to care.

7. Oversees dialysis technicians in the performance of dialysis from start to finish, monitors patient reaction to treatment and performance of the dialysis machines, and demonstrates an in-depth understanding of the mechanics of dialysis.

8. Assesses, prepares, and cannulates a patient' s vascular access (including needle insertion in arterioveinous fistula or grafts for aseptic connection with dialysis equipment).

9. Utilizes sterile techniques to assess the dialysis catheter exit site and to apply dressing on the patient' s vascular access (dialysis catheter , arterioveinous access) per protocols.

10. Maintains skills necessary to perform surveillance on dialysis access (catheter, fistula, graft) for early signs of failure or infection; this includes (if needed and after receiving the appropriate training) the ability to perform arterioveinous access monitoring using ultrasound machine per protocol.

11. Reviews patient' s electronic/paper records for orders as appropriate and administers dialysisrelated intra-venous and/or oral medications as prescribed and/or per protocol.

12. Conducts post dialysis assessment and patient discharge by evaluating treatment, documenting medication administration utilizing the appropriate protocols, assessing condition of vascular access, discharging the patient when vital signs are stable, and documenting and recording patient treatment data.

13. Provides patient education/teaching applicable to the patient' s medical condition.

14. Refers patient to other disciplines or services as needed (nutrition, social work, etc).

15. Performs other treatments and diagnostic tests such as blood sugar testing and other tests required as part of patient treatment.

16. Oversees disassembly and disinfection of dialysis equipment.

17. Maintains skills necessary to do regional heparinization, separate and store blood samples, set up oxygen, administer saline, and maintain dialysis equipment.

18. Performs surveillance and testing of dialysis water treatment equipment and dialysis water (such as chlorine, hardness, conductivity, water cultures) to ensure that these machines are operating within the parameters set in the facility's written protocols.

19. Demonstrates a working knowledge of dialysis laboratory tests and dialysis medications.

20. Demonstrates an in-depth understanding of infection control and prevention of transmission of blood borne pathogens.

21. Participates in quality improvement activities to include collection of data for the dialysis unit Quality Assurance Performance Improvement (QAPI) plan.

22. Responds appropriately and provides assistance in emergency situations.

23. Provides leadership in improving patient care, maintaining standards of care, directing and teaching personnel in a variety of settings, and providing charge nurse responsibilities as needed.

24. Works closely with others on the dialysis team and participates in interdisciplinary team meetings.

25. Maintains responsibility for providing coverage to the dialysis department.

JOB DESCRIPTION:

• Prepares patients prior (checks patients' vital signs, laboratory results, fluid balances), during (monitors, adjusts treatment settings, administer and carry out doctor's orders, medications), and after the procedure (post-procedural care, home medications including instructions);
• Keeps accurate records of treatment, patient status, and any change in patient condition, including the documentation of patient and family education on continuity of care and DOH-mandated registries;
• Shares important information with doctors and other members of the health care team to provide the best care for patients;
• Observes infection control guidelines, uses aseptic techniques especially when handling hemodialysis access sites;
• Responds to emergencies and other hemodialysis complications; performs CPR, first aid when necessary;

• Sets up, operates, troubleshoots, cleans hemodialysis machines and accessories such as dialyzers, reverse osmosis system, and reports any equipment concerns to both Biomed and suppliers; assists with repairs whenever necessary;
• Facilitates charges needed in hemodialysis treatment of patient including the Philhealth; and
• Performs other duties that may be assigned by the Immediate Superior from time to time.

JOB QUALIFICATIONS:

• Graduate of Bachelor of Science in Nursing
• At least six (6) months experience in hospital wards or similar field
