team as
Social Worker,
where you will be responsible for:

  • Empowering vulnerable young people to access and navigate primary health services through inclusive engagement and tailored case management.
  • Delivering high-quality youth health care aligned with the Metropolitan Youth Health Model of Care, including intake, assessment, referral, and psychosocial support.
  • Collaborating with families, health professionals, and community agencies to strengthen service access and improve long-term outcomes for young people, especially young parents.
  • Leading continuous improvement through research, evaluation, and the development of youth-focused policies, programs, and clinical practices.
  • Providing professional supervision and mentorship to staff and students, supporting a culture of growth, accountability, and trauma-informed care.

About MYH

  • MYH works on prevention, early intervention and youth specialist health response measures regarding adverse childhood events (ACE) and their impact on young people's health, including speciality programs for young parents
  • This position is located at the clinical health service location of MYH West (which is also the home base for the MYH Young Parents. Program), where multiple support programs run, including the Talking Realities FLO Program and various grant funded Young Parents support Programs.
  • MYH programs at this site, focus on building and strengthening safe and positive relationships for all young people accessing our health services but with special focus on young parents and their children.
  • Building these protective factors improve children and young people's resilience and reduce the impacts of ACE's on their future health and disrupting intergenerational cycles.

About You
To be successful in this role, you will have:

  • Degree in Social Work (or equivalent) with eligibility for full membership of the Australian Association of Social Workers.
  • Proven experience delivering youth-focused health care, including assessment, case planning, psychosocial support, and advocacy.
  • Strong knowledge of adolescent development, youth mental health, and social determinants of health, particularly for priority populations.
  • Demonstrated ability to work collaboratively within multi-disciplinary teams, communicate across diverse backgrounds, and manage competing priorities.
  • Experience in facilitating group programs and contributing to service improvement through training, evaluation, and quality initiatives.
  • Skilled in supervising and mentoring staff or students, with a commitment to continuous learning and best practice.

Position Summary: We are seeking a seasoned, empathetic, and highly organized Case Manager to join our social work team. The ideal candidate will have extensive experience working with young girls facing complex trauma, exploitation, early motherhood, daily challenges, and survival-based employment. You will manage a caseload, develop individualized service plans, and coordinate resources to promote safety, healing, education, and empowerment for each client.

Key Responsibilities:

  • Conduct comprehensive assessments to determine client needs, risks, strengths, and goals.

Duties and Responsibilities Aligns all functions with the mission and vision of The Camillian Fathers, Inc., St. Camillus Polyclinic Charity Foundation, and the Philanthropic Development Office (PDO). Establishes and maintains professional and systematic donor–donee relations with local and international partners (e.g., Camillian Giving Circle, PCNC, etc.) to support Camillian advocacies.
