We are looking for a Registered/Dialysis Nurse to care for our patients and facilitate their speedy recovery. You will also be responsible for educating them and their families on prevention and healthy habits.

The ideal candidate must be a responsible and well-trained professional able to give the best nursing care with little supervision. You should be compassionate and cool-tempered. You will follow health and safety guidelines faithfully and consistently.

The goal is to promote patient's well-being by providing high quality nursing care.

Responsibilities

Monitor patient's condition and assess their needs to provide the best possible care and advice

Observe and interpret patient's symptoms and communicate them to physicians

Collaborate with physicians and nurses to devise individualized care plans for patients

Perform routine procedures (blood pressure measurements, administering injections etc.) and fill in patients' charts

Adjust and administer patient's medication and provide treatments according to physician's orders

Inspect the facilities and act to maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds etc.)

Provide instant medical care in emergencies

Supervise and train nursing assistants

Foster a supportive and compassionate environment to care for patients and their families

Expand knowledge and capabilities by attending educational workshops, conferences etc.

Requirements and skills

Proven experience as a registered nurse

Excellent knowledge of nursing care methods and procedures

Excellent knowledge of emergency care

In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times

A team player with excellent communication and interpersonal skills

Responsible and compassionate

BSc or other diploma from a nursing program

FUNCTIONS

I. DIRECT PATIENT CARE

• Performs direct nursing care procedures, which include bedside nursing, hygiene, pre and post-operative care, and admission and discharge duties in accordance to the standards of nursing practice and hospital policies.
• Plans nursing care appropriately by involving the patient/family utilizing the Nursing Process (Assessment, Diagnosis/Analysis, Planning, Intervention, and Evaluation).
• Prepares patients for and assists physicians with diagnostic and therapeutic procedures.
• Administers medications and other therapeutics observing the 10 R's of safe medication administration.
• Refers patient's condition to Attending Physician observing proper referral procedure with complete information (Patient's Name, Diagnosis, Room Admitted, Observations Noted, Latest Vital Signs, Nursing Management given) and details.
• Carries out Doctor's legal orders.
• Maintains a safe and therapeutic environment of the patient by ensuring compliance to hospital policies and safety standards.

II. HEALTH EDUCATION AND PATIENT ADVOCACY

• Assists in the education and rehabilitation of patients and their families related to physical and mental health.
• Acts as liaison between patients and hospital personnel.
• Orientation of Patient's Rights and Responsibilities to patients assigned to him/her.

III. RECORDS AND DOCUMENTATION

• Charts accurately all observations, decisions, actions, and outcome of actions rendered to patients thru the utilization of the FDAR mode of documentation taking into consideration the time element.
• Monitors the inventory and maintenance of supplies, equipment and facilities, and ensures judicious use of supplies and materials.

OTHER FUNCTIONS

• Coordinates with other services in the hospital regarding patient care needs.
• Attends regular unit meetings and other training programs required.
• Performs other related functions as maybe assigned.

• Provides direct care to patients undergoing HD based on the rules and regulations established by DOH and approved company policies and procedures.
• Checks and prepare all materials needed and ensure it is available before the start of dialysis.
• Reads and carries out doctors' orders and ensures that dialysis is performed accordingly.

• Updates patient's records during and after every dialysis session.
• Inventories e-cart medicines and ward stocks at the end of every shift.
• Protects patients from accidents, hazards and potential infections.
• Observes the line of communication and applies appropriate communication techniques in reporting, recording and relaying information or data.
• Provides information and health education to patients and significant others regarding the plan of care.

JOB QUALIFICATION

• Registered Nurse
• Must have a certificate of training in the nursing care of renal dialysis patients from competent training providers.
• Must have a certificate of training in Basic Life Support (BLS) conducted by competent authorities or professional organizations.
