br>Key Responsibilities:
Deliver direct nursing care to patients requiring long-term acute care, including ventilator support, complex wound care, and IV therapy.
Monitor and assess patients’ conditions, manage symptoms, and report changes in status to the care team. < r>
Collaborate with multidisciplinary teams including physicians, physical therapists, respiratory therapists, and case managers to create and implement care plans.
Educate patients and families on recovery goals, discharge planning, and health maintenance.
Accurately document all care activities in the electronic medical record (EMR) in compliance with facility policies and regulatory standards.
Ensure strict adherence to infection control practices, medication safety protocols, and patient confidentiality (HIPAA).
Qualifications:
Active USRN license (state-specific or compact license as applicable).
Minimum 1 year of bedside experience in acute care, LTAC, or rehabilitation settings preferred.
BLS and ACLS certifications required.
Strong clinical judgment, communication, and critical thinking skills.

Job Description
Under the direct supervision of the Senior Physical Therapist, responsible for providing physical therapy services as determined by medical direction, therapy judgment, patient needs and desires, and available facilities.
**SPECIFIC DUTIES AND RESPONSIBILITIES**
1. Ensures completeness of all needed supplies and proper maintenance of all machine and equipment in the PT Department
- Maintains the cleanliness and orderliness of the department
- Ensures that preventive maintenance for equipment/machine is being conducted regularly
- Ensures that all PT supplies are available at all times
- Monitors and controls usage of supplies in the department
2. Correctly performs all PT procedures/modalities that are recommended to patient
- Establishes rapport with patient upon admission to the department
- Assesses patients problem and formulate appropriate treatment plan in accordance with the Physiatris’s orders
- Initiates PT program upon MD referral with applicable diagnosis and orders
- Properly administers PT procedure and observes precautionary measures to ensure safety of the patient throughout the treatment plan
- Observes proper draping and respects patient’s privacy
- Assesses/evaluates patient progress and alters treatment plan as needed in coordination with the Physiatrist
- Evaluates level of dependence or difficulty during position changes and provides stability and/or mobility training to enhance function at work or at home
- Assesses the ergonomic factors at home or at work and enjoins correction of unsafe functional skills including proper lifting and body mechanics
- Suggests behavioral coping strategies in management of transient and permanent disability
- Encourages patient and family compliance to home program throughout the course of patient’s treatment and after patient’s discharge
- Accurately computes PT charges and direct patient to cashier for payment
3. Strictly applies the principle in infectious control and observes universal precaution to prevent contamination
4. Maintains patient PT records, charts and discharges summaries, and maintain strictest confidentiality of same
5. Attends regular staff meeting
6. Participates in a monthly staff development program
7. Submits required reports to clinic Manager/QA Manager
8. Performs other related tasks assigned by the Senior Physical Therapist/Clinic Officer from time to time
**JOB SPECIFICATION**
- Graduate of a reputable/accredited school
- Graduate of BS Physical Therapy, licensed
- Must have sound knowledge in performing different PT procedures
- Good communication skills in both oral and written English and Filipino
- Computer literate, with knowledge of MS Windows
