- Obtains medical history through intake and assessments, including measuring of vital signs.
- Provides information to patients and their families about care plans and procedures.
- Provides emotional support to patients and their families
Clinical Responsibilities
- Maintains cleanliness of patient care areas and adhere to all safety protocols, infection control procedures and facility policies.
- Participates in handling and disposal of hazardous materials
- Maintains professional relationship with all members of the healthcare team
- Performs other clinical operation functions that may be assigned from time to time
Communication and Documentation
- Accurate documentation of patient information and medical history; and encoding the same via available system.

- Prepares, reconciles and submits census and billing related reports to respective units.
Patient Education and Continuous Learning and Development
- Educates patient and families on care procedures and health management
- Participates in patient care planning and team meetings as needed.
- Participates in staff training and development program as required
QUALIFICATIONS/SKILLS REQUIRED:
- Preferably College degree graduate/s (medical field is a plus).
- If not college graduate, at least 2 years college or Senior High graduate
- Prior experience in a healthcare setting or customer service is highly desirable
- PCT Certification or Health Care Services NC II with or without certificate is a plus but not required.
- Must be computer literate and can operate basic clinical tools/equipment
- Personable with good communication skills and empathetic skill-sets
- Excellent organizational and multi-tasking abilities
- Strong interpersonal skills and ability to work as part of a team
